We are currently collecting impact stories from CHWs, CHW clients and those who work with CHWs in their organizations. We want to highlight the important work of CHWs including:
Addressing behavioral health care
Serving aging adults
Providing care in rural Montana
Connecting clients to community resources, including Medicaid and charity care
Increasing trust in our systems
Improving health outcomes
Extending our clinical workforce

Yellowstone County has a rich network of CHWs across a variety of different organizations including: Chamber of Commerce, Public Health Department, Urban Indian Organization, Hospital, and Aging and Disabilities non-profit.
